Eyebag surgery can be performed via inside the eyelid (transconjunctival) or outside the eyelid (transcutaneous) routes. Both methods will be combined with fat transposition. 

Transconjunctival approach is scarless as the incision is made from inside the eyelid. This technique is usually suitable for younger patient without excess and sagging lower eyelid skin. Any excess skin can be addressed using skin pinch method, but it is not as powerful as a midface lift. 

Transcutaneous approach involves an incision the lower eyelid just below the lashes. This technique can be combined with a midface lift to address the excess and sagging lower eyelid skin.
